If you wish to become a member of the Plan and join the Superannuation Scheme, the KiwiSaver Scheme or both, the application forms and Scheme details are below:
Superannuation Scheme
Application Form
This is the form that is to be filled in by an employee who wishes to become a member of the Scheme. The form requires the employee to elect a contribution rate and to nominate possible beneficiaries, and authorises the employer to make the necessary payroll deductions.
Investment Statement
The Investment Statement sets out in some detail information about the investments of the Scheme. The Statement follows legal requirements and provides information about general investment decisions such as choosing an investment and choosing an investment advisor, as well as providing other important information about the Scheme.
Beneficiary Form
This is the form that allows a member to elect or to change those who are nominated as possible beneficiaries in the event of the death of a member. The information on the form will be used by the Trustees to guide them in the payment of a death benefit from the Scheme.
